Crush Request Form

This form has been disabled, and moved to within your license management account.

Please log into your account, then follow these steps to submit a “crush” request and have the machine ID of a broken or lost computer removed:

  • Click “Your Software Licenses”
  • Click the License Code that the computer is registered to
  • Click [Release] next to the machine ID of the computer to be removed
  • Click “REQUEST MACHINE ID REMOVAL HERE” at the bottom of the page
  • Follow the directions and fill out the form
